If you or your family is struggling to afford something that is needed to overcome a major medical diagnosis, or if you know a family that is in need, please fill out the form below. One Catalyst grants typically have a value up to $2,000.

The One Catalyst Grant Application

Founded in 2016, One Catalyst Foundation is a 501(c)(3) organization dedicated to providing tangible support to the members of our community as well as medical financial assistance. Through fundraisers, events, and donations, One Catalyst is able to ease the burden felt by individuals suffering from a medical condition that are in need of financial assistance.

Purpose

The Grant award goes to qualified individuals who are in need of financial assistance due to a documented medical condition.

Eligibility

The Grant is eligible to individuals who are suffering from a medical condition from which financial assistance is required to help ease the individual’s burden. Grant recipients must be able to provide proof of medical condition and treatment with letter of confirmation by a physician.

Applicant must be a US citizen and legally reside in New Jersey, New York, Pennsylvania or Connecticut.

Submission Materials– All items must be submitted to be considered eligible.

All checks are paid to the individual directly. You will only receive communication from One Catalyst if you have been awarded the grant.
